Waiver and Release of Liability

1. Acknowledgment of Risk I, the undersigned, understand that participation in activities within JASE involves engagement in sensory-focused environments, specialized equipment, and other interactive experiences, which may involve inherent risks, including but not limited to physical injury, sensory overload, or property damage. I voluntarily assume all risks associated with my or my child’s participation in activities at JASE.

2. Release of Liability In consideration of being permitted to use JASE’s facilities, I, on behalf of myself, my child (if applicable), my heirs, executors, administrators, and assigns, release, discharge, and hold harmless JASE, its owners, staff, volunteers, agents, and affiliates from any and all liability for any injury, loss, or damage arising out of or in connection with my or my child’s participation in activities at JASE, except as caused by the gross negligence or willful misconduct of JASE.

3. Indemnification I agree to indemnify and hold harmless JASE, its owners, employees, volunteers, agents, and affiliates from any claims, actions, suits, or demands made against them, including reasonable legal fees, resulting from my or my child’s actions or use of the event space and equipment.

4. Use of Premises and Equipment I agree to abide by all rules and guidelines provided by JASE, including appropriate use of sensory equipment, quiet zones, and other facilities. I understand that any damage to the property caused by myself, my child, or members of my party will be my responsibility.

5. Media Release (Optional) I grant permission for JASE to use photographs, video recordings, or other media of myself or my child taken during events for promotional purposes.

6. Emergency Medical Treatment In the event of a medical emergency, I authorize JASE staff to seek medical assistance on my or my child’s behalf. I agree to bear all costs associated with such treatment and understand that JASE staff are not medical professionals.

7. Governing Law This agreement shall be governed by the laws of the State of Florida. Any disputes arising under this agreement shall be resolved in a court of competent jurisdiction in Broward County, Florida. By submitting this form, I confirm that I have read and fully understand this waiver, and I voluntarily agree to its terms.
